Things you should know before you travel to our main office:

CSBPS is located in North Seattle in the Northgate Plaza Building. The building is less than a mile South of Northgate Mall on the street just East of I-5. The Northgate Plaza Building is a five-story brown building with green glass and sits behind Marie Callendar's restaurant.

The building is accessed via the first driveway South of the restaurant. The entrance is in the middle of the building and visitor parking spaces are available in the rows next to the building on the North and South sides. Handicap parking is available near each entrance. CSBPS is on the first floor in the Southeast corner in Suite 100.

There is 2 hour visitor parking on the South side of the building, which is closest to our agency entrance. Once in the main entrance we are located in Suite 100 to your right.

There is 30 minute visitor parking on the North side of the building. It is just a quick walk through the main lobby. We are located just past the elevators on the left hand side.

Agency Hours: Monday through Friday from 8:00 a.m. to 5:00 p.m.

By Car from I-5

From the North or South, take Exit 173 (Northgate Way, First Ave NE).
From the South, at the stoplight turn right (South) onto First Ave NE.
From the North, stay to the left on the exit ramp and follow signs to Northgate Way. At Northgate Way turn right (East) and pass under the freeway. Turn right (South) onto First Ave NE.
Drive South on First Ave NE past the Northgate Mall, the Northgate Transit Center and Northgate Station (SAS Shoes and Silver Platters are two tenants to watch for as landmarks.)
Drive by Marie Callendar’s restaurant, then immediately turn left into the driveway marked "Northgate Plaza." (NOTE: This sign can be hard to see due to surrounding foliage.)
By Car from the North or South on Surface Streets Fifth Ave NE is the main North/South arterial between NE 80th and NE 175th St.
Travel North or South on Fifth Ave NE and turn west on NE 100th St. (Group Health has a large building on the corner.)
Travel three blocks west and turn left (South) onto First Ave NE. You will pass Northgate Station (SAS Shoes and Silver Platters are two tenants to watch for as landmarks.)
Drive by Marie Callendar's restaurant, then immediately turn left into the driveway marked "Northgate Plaza." (NOTE: This sign can be hard to see due to surrounding foliage.)

By Bus (King County Metro)

The Northgate Transit Center is at 10200 First Ave NE, within a few blocks of CSBPS.

By Car

The Sedro Woolley clinic is held in North Cascade Eye Associates office, which is conveniently located across the street from United General Hospital, in North Cascade Professional Park, just off SR 20.

From the North take I-5 South to the Cook Road exit. Turn left at the stop sign onto Cook Road. Travel East on Cook Road, approximately 3.5 miles to Collins Road. Turn Right onto Collins Road and continue South just under 1 mile to Hospital Drive. Turn Left onto Hospital Drive. 2131 Hospital Drive is on your Right just across from the main entrance to the Hospital.
From the West and South take I-5 North to the Cook Road Exit. Turn Right onto Cook Road. Follow the directions above once you have made the turn onto Cook Road.

From the East take SR 20 through Sedro Woolley. From the junction with SR 9 continue West on SR 20 for approximately 2.5 miles. Turn Right at the sign for United General Hospital. 2131 Hospital Dr. is you Left just across from the main entrance to the hospital.

By Car

The Bremerton clinic is held in Kitsap Eye Physicians Offices.

From the North take SR 3 South to the East Bremerton exit. This exit will become SR 303 South. Stay on SR 303 for approximately 8.5 miles. SR 303 becomes NE WAAGA Way/SR 303. After about 4 miles SR 303/WAAGA Way becomes Wheaton Way. Continue South on Wheaton Way until you reach Sheridan Road. Turn Left onto Sheridan Rd. Then turn immediately Right onto (Old) Wheaton Way. Kitsap Eye Physicians is on the corner of (Old) Wheaton Way and Callahan Drive. The office is nearby Harrison Hospital in Bremerton.
From the South take SR 3 North to the Kitsap Way/SR 310 exit. Turn Right onto Kitsap Way. Continue on Kitsap Way to 11th Street. Turn left onto 11th Street and continue to Warren Ave/SR 303. Turn Left onto Warren Ave. Warren Ave/SR 303 becomes Warren Avenue Bridge/SR 303. Warren Ave Bridge becomes SR 303. Take the Callahan Drive off ramp and merge onto Callahan Drive. Kitsap Eye Physicians is on your Right on the corner of (Old) Wheaton Way and Callahan Drive.
